Projeto Terapêutico Singular em um município do interior paulista: um estudo de caso

Abstract

This work aims to present the case study of one of the participants of the project Unique Therapeutic (PTS) of the municipality of Brejo Alegre, developed in the year 2015. The case report was based on the stages of construction of the PTS deployed in the city. 1st step: realization of diagnostics and user analysis, leading to the conclusion about the risks, vulnerabilities and potential. In step 2 were defined the actions and goals. The third step was performed the monitoring and reassessment of the case. Results: the greatest benefits achieved with the participation in PTS were the improvements obtained physical (functional independence), emotional and social, in addition to the innovation of an interdisciplinary team to discuss the case. The results of user presented demonstrate the importance of the practice of the PTS for basic care of the municipality of Brejo Alegre, making the use of this instrument becomes part of the routine of this team.
