Abstract
Why some communities thrive, have effective political institutions and healthy citizens, while other communities do not? Why sharp differences in overall mortality and for specific causes, such as diseases cardiovascular disease, cancer and violence,according to the level socioeconomic status, are observed both in countries with low levels of social inequality, as well as in
those with universal health coverage?
