Resumo
Depois de breve revisão sobre a literatura brasileira referente às shigeloses, o autor descreve os métodos e o material que usou na execução deste trabalho. Usou, como meios de cultura para isolamento de bacilos disentéricos, o ágar S. S. e o ágar de Holt, Harris e Teague. Na identificação das bactérias isoladas, empregou provas bioquímicas usuais e, em seguida, aglutinação com soros polivalentes e, quando necessário, soros monovalentes. O método de obtenção desses soros, assim como a técnica da aglutinação foram descritas detalhadamente. Não pretendendo discutir questões de sistemática bacteriana, divide os bacilos disentéricos em 3 grupos:
1o grupo: manita + lactose - Sh. paradysenteriae, Sh. alkalescens, Sh. tietê;
2° grupo: manita + lactose - Sh. sonnei, Sh. díspar;
3° grupo: manita - lactose + Sh. dysenteriae, Sh. ambígua, Sh. sp, grupo Large-Sachs.
Estudando cada grupo isoladamente, adota o critério de J. S. K. Boyd modificado por K. M. Wheeler para diferenciação dos vários tipos do grupo paradisentérico. Neste grupo, não conseguiu encontrar diferenças antigênicas entre Sh. paradysenteriae tipo IV e com o tipo descrito como Sh. saigon e, posteriormente, como Sh. rio, a não ser em pequenas variações nos componentes antigênicos de grupo. Adota o critério de Assis, separando a Sh. tietê da Sh. alkalescens; aceitando a existência, neste grupo, de variantes acidificadoras da lactose. Estuda o grupo manita-negativa, aceitando, em parte, como bacilos disentéricos, os tipos descritos por Large e Sachs. O material estudado pode ser dividido em 3 grupos:
1o Pesquisa de bacilos disentéricos em 307 crianças, comparando o método do bastão retal com o da semeadura das fezes. Os resultados foram os seguintes: bastão retal 75% de culturas positivas e cultura de fezes 57%; as bactérias isoladas foram as seguintes: Sh. sonnei (12), Sh. paradysenteriae (9), Sh. alkalescens (6), Sh. ambigua (1); dentre estes, houve 5 casos fatais: Sh. paradysenteriae (1), Sh, sonnei (2), Sh. alkalescens (2).
2o Estuda os resultados de 5.211 coproculturas realizadas num período de 4 anos em fezes com suspeita ou não de shigelose. O número de exames positivos foi de 497 e as espécies encontradas foram as seguintes: Sh. paradysenteriae tipo I (16), tipo II (97), tipo III (30), tipo IV (28), tipo V (11), tipo VI (20), tipo VII (4), tipo VIII 2), não tipadas (18); Sh. alkalescens (73), Sh. tietê (50), Sh. sonnei (100), Sh. dysenteriae (6), Sh. ambigua (14), Sh. sp. Sachs Q 771 (3), Sh. sp. Sachs Q 454 (4).
3o Estuda a incidência de bacilos disentéricos nas fezes de 100 doentes portadores de enterolites crônicas. Em 23 doentes, pôde evidenciar a presença de shigelas nas fezes; as espécies isoladas foram as seguintes: Sh. paradysenteriae (7), Sh. sonnei (3), Sh. alkcalescens (8), Sh. dispar (5), sendo que, em 2 casos, havia 2 bacilos disentéricos. O número de exames feitos nestes doentes variou de 2 a 15, com intervalos em geral de 1 semana, sendo interessante notar períodos longos em que o germe não podia ser evidenciado.
