Recurring problems in low therapeutic index pharmacon manipulation

Thyroid hormones as thyroxine (T4) and liothyronine (T3) are important drugs for the treatment of hypothyroidism, deficiency caused by low levels of T3 and/or T4 in the body. The Antibiotics Laboratory of of Adolfo Lutz Institute from São Paulo received many complaints of compounded drug formulas containing these hormones. The objective of this research was to report drug samples containing T3 and/or T4 suspected of quality deviations related to adverse events, originated from compounded pharmacies of the cities in the State of São Paulo State. One of the samples revealed serious errors in the compounded process of the product presenting a milligram dose , when the usual therapeutic doses of T4 and T3 are prescribed in microgram. Large variability has been found between capsules of the same flask and the lack of uniformity in the samples analyzed, featuring serious quality deviations in the compounded process of the product. The results showed fluctuations between ineffective therapeutic doses and overdoses, indicating not only the lack of implementation of the Good Compounding Practices (GCP) according to Resolution RDC No. 67 as well as the risks that consumers of this type of product are exposed.
