Resumo
Foi analisada a contaminação por enterobactérias e seus mecanismos de introdução e manutenção em 12 berçários do Município de São Paulo, no período de janeiro a março de 1976. Foram examinadas 4.001 amostras obtidas de alimento, do leito subungueal, de fezes e do meio ambiente. Detectou-se contaminação fecal em grau variável no universo de todos os berçários, inclusive de centros obstétricos e lactários. A variação percentual da contaminação permitiu a divisão dos berçários em dois grupos: o primeiro, constituído por sete berçários com nível de contaminação de 4,9% até 8,7% e o segundo, variando de 14,4% a 25,2%. O achado de 38 cepas de enterobactérias patogênicas nas mais variadas localizações do meio ambiente, em metade dos berçários, indica que estes enteropatógenos pertenciam à microbiota normal, pois estes berçários funcionavam normalmente e, no momento da pesquisa, não havia denúncia de surtos epidêmicos. O percentual médio de positividade de contaminação fecal do leito subungueal dos funcionários foi de 24,6%. O encontro de enteropatógenos nas fezes de recém-nascidos apresentava elevação do percentual de positividade à medida em que aumentava a sua permanência nos berçários. Em nosso meio, os enteropatógenos isolados das fezes de funcionários carecem, no momento, de importância epidemiológica, pois os seus sorotipos são diferentes dos encontrados no meio ambiente e nas fezes de recém-nascidos. A análise dos perfis globais de resistência e sensibilidade aos vários antimicrobianos testados permitiu a divisão das estirpes de bactérias enteropatogênicas em dois grupos com importância epidemiológica: o das linhagens hospitalares, cujo principal representante é Salmonella typhimurium, e o das cepas de passagem, como S. agona, S. minnesota, S. neurport., S. infantis e outras.
